Instructions and Forms

 

Following are the documents used in the application for a KFF Grant process.
To be able to submit these documents you need to
1. Register your organization with the KFF 

2. Be invited by the KFF e-mail to proceed with the application
3. Receive these documents in the Word format from the KFF 

Step 1: LOI - Letter of Inquiry
First document in the application process that you will receive. You can submit LOI at any time. After it is received by the KFF, you will be notified within six to eight weeks (grants are reviewed at KFF meetings every other month) whether the grant is awarded right away or the KFF needs more information and a more detailed Application Form is requested in Step 2. If the grant is awarded, go to Step 3. 
Step 2: Application Form
The KFF decision is based upon your LOI. If the Application Form is requested you will receive an e-mail. You can then submit the Application at any time. Grants are reviewed at KFF meetings every other month unless specified in the instructions of a specific program. If the grant is awarded, go to Step 3.
Step 3: End of the Grant Report
After the grant is awarded, you will get a Grant Award letter and a check. You will also be asked to submit End of the Grant Report and given a deadline.While most programs likely will not require an outside, formal evaluation, we do expect a thoughtful evaluative process and useful product.
Step 4: If you are a Returning Grantee, go to "Start with Registration"

The following KFF forms in PDF format  (except for the End of the KFF Grant Report) are for informational purposes only. In order to qualify for a grant you will have to follow the application process starting by registering your organization with the KFF.
